Understanding Concussions in Soccer: Risks, Recovery, and the Role of Physical Therapy

A concussion is a type of traumatic brain injury caused by a forceful blow to the head or body that makes the brain shake within the skull. This disruption can cause a range of symptoms, including headaches, dizziness, blurred vision, and difficulties with concentration and memory. Concussions are particularly prevalent in contact sports like soccer, where players frequently head the ball, collide with opponents, or fall to the ground. While many people associate concussions with more violent contact sports like football or hockey, soccer players also face a significant risk of head injuries due to the repetitive impacts they endure during games and practice.

Concussions in soccer are often underreported or go unnoticed, as symptoms can sometimes be subtle or develop later. Athletes may not lose consciousness, but they might experience confusion, light sensitivity, or emotional instability. Moreover, soccer players are at risk for subconcussive impacts, which are repetitive head traumas that don’t produce immediate, obvious symptoms but can lead to long-term neurological issues. Over time, these impacts can accumulate and contribute to cognitive decline, mood disorders, and conditions like chronic traumatic encephalopathy (CTE).

Physical therapy plays a vital role in the rehabilitation of athletes recovering from concussions. A key element in recovery is the “graduated return to play” protocol, which ensures that athletes reintroduce physical activity safely and progressively. This step-by-step approach starts with complete physical and cognitive rest, giving the brain time to heal from the initial injury. Once symptoms subside, light aerobic exercise such as walking or stationary cycling is introduced to increase the heart rate without placing too much stress on the brain.

In soccer, athletes may progress to sport-specific drills like running without engaging in heading or contact activities. As they demonstrate the ability to handle more complex movements without symptoms, players can then participate in non-contact drills and resistance training. Full-contact practice is only permitted once the athlete has received medical clearance and has demonstrated the ability to perform at full intensity without a recurrence of symptoms.

The final stage of the return-to-play protocol allows the athlete to return to normal game play. This careful progression ensures that athletes do not rush back into competition before they are fully healed, reducing the risk of further injury or long-term complications. By following a structured recovery plan with a physical therapist, soccer players can safely return to their sport while protecting their long-term brain health.
